HEALTH DEPARTMENT REQUIREMENTS FOR TEMPORARY LICENSE

Instructions for an Application for a
Temporary Permit to Practice Body Art 
in the City of Worcester

Please make sure you are familiar with Worcester Regulations for Body Art: Regulations

  1. Complete the Application: Download Application Here
  2. Provide a check or money order for $40.00. Please make checks payable to: City of Worcester.
  3. Provide documentation of:
    • Current CPR certification www.icpri.com)
    • Current First Aid certification www.icpri.com)
    • Current Blood Bourne Pathogens certification (www.icpri.com)
    • Anatomy Course certificate for Piercers
    • Valid Body Art License or Certificate for other City or State

(All documentation subject to verification)

Mail the application package with the appropriate application fee to:

We have a room block at the Hilton Garden Inn (www.worcester.hgi.com) which is a 1/2 block down the street from the DCU center.  Our discounted room rate is $129 a night, just let them know you are with the Massachusetts Tattoo Convention.
